I, the undersigned participant, acknowledge that participating in the Golf Swing Speed Contest involves physical activity that may result in injury or harm. I voluntarily assume all risks associated with my participation in this event. In consideration of being allowed to participate, I hereby release and hold harmless Streamline, its employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage I may sustain during the event. I further acknowledge that I am participating voluntarily, and I certify that I am physically capable of participating in this activity. If I experience any physical discomfort or injury, I will immediately discontinue my participation. By signing below, I confirm that I have read and understood this waiver, and I agree to its terms.
